Eligibility Guidelines
There are three requirements that must be met to be eligible to shop at Outfitters:
- You must attend, or live within the boundaries of, the South Madison Community School Corporation
- Your children must be school-age (Kindergarten-12th grade)
- Your family must be considered in financial need of assistance, such as:
- On the free or reduced lunch program in South Madison Community School Corporation
- Receiving services from another social service agency or
- In crisis: fire, flood, lost job, medical reasons, family disruption, etc.
The following persons can refer a family to Outfitters:
- A schoolteacher, guidance counselor, social worker, or school system administrator
- A pastor, priest or rabbi from your place of worship
- A social worker/caseworker employed at a local social service agency, a township trustee in South Madison County, police or fire department personnel
To shop at Outfitters, you must:
Get a completed referral form from:
- A school
- A church
- A caseworker, township trustee, or police/fire department personnel
- Or use an Outfitters Free Lunch Voucher issued from the South Madison Community School Corporation
A completed Referral Form or Free Lunch Voucher entitles each child to shop at Outfitters in the Fall and Spring. A new referral form must be obtained at the beginning of each school year.
Shopping is by appointment only. Referred families should call Outfitters at (765) 778-9097 for an appointment.
